What is YourMedChart?
YourMedChart is an online health forum in which patients can manage their healthcare information by updating their healthcare profiles and archiving official medical records from doctors' offices and hospitals. YourMedChart securely puts these records online so they are accessible from anywhere in the world at anytime. Access to a patient's record is password-protected and only available to doctors authorized by the patient. Your privacy is our utmost concern.
How does it work?
YourMedChart is a web-based application in which medical information previously submitted by the patient is made available. Physicians and healthcare professionals at YourMedChart organize submitted reports and images into classifications that are used in medical practice. This organization makes patients' online records more useful to their physicians. Information is encrypted when sent over the Internet and kept secure with access only granted to authorized physicians that a patient chooses.
There are multiple ways for patients to get information into their online record (see Instructions). All of the information is organized for easy viewing.
